EARLY CENTRAL AC & HEAT PUMP REPLACEMENT REBATE — TERMS AND CONDITIONS
Existing System Requirements: Central Air Conditioning (AC) or electric Air Source Heat Pumps (ASHP) units/systems must be verified as 12
years or older and functioning. Age and functionality can either be confirmed by a Mass Save Energy Home Energy Service Program site visit, or
by having a COOL SMART® QIV Trained Contractor verify and perform an Early Replacement QIV Pre-test.
New System Requirements: Central AC or electric ASHP units/systems must be qualified, listed with and certified by the Air Conditioning,
Heating, and Refrigeration Institute (AHRI), and meet the program SEER, EER and HSPF requirements. The AC/ASHP condenser and the
evaporative coil must be new and replaced together. The condenser and coil are separate components in a split AC or ASHP system and,
for rebate purposes, these are considered one unit.
Proof of Installation: Customers must submit this rebate form along with all supporting documents as stated on the front page.
Information Sources to Verify Equipment Eligibility: SEER, EER ratings (for AC only) and SEER, HSPF ratings (for heat pumps only) for
condenser, evaporator and air handler (if applicable) must be provided. Both the Consortium for Energy Efficiency () and
the AHRI directory () web sites list SEER, EER and HSPF values. AHRI also provides AHRI numbers. Manufacturer’s spec sheets
may be accepted ONLY if equipment is not yet AHRI rated and ONLY if AHRI listing is pending.
Customer Eligibility Requirements: Customer must reside in a 1-4 family home. Existing equipment must be deemed eligible by approved
verifier (COOL SMART QIV Trained Contractor or Home Energy Specialist) between April 1, 2015 and August 31, 2015, and installed by
October 30, 2015. Application must be postmarked by October 30, 2015.
Geographic Requirements: Offers valid only for Cape Light Compact, Eversource and Unitil residential electric customers in MA. Municipal
and National Grid electric customers are not eligible.
Rebate Application Form: This application must be filled out completely, truthfully, and accurately. The customer and contractor must sign and
date this form.
Quality Installation Verification (QIV) Rebate: For customer to qualify for $150 QIV Rebate, each qualifying system must be installed and properly
tested by a COOL SMART QIV Trained Contractor listed on The additional rebate applies to each qualifying new system
tested and may be mailed separately from the Early AC/ASHP Replacement rebates. Contractor must verify proper refrigerant charge and
operating capacity of the unit. Test conditions must be 60°F+ Outdoor (not raining), 65+F Indoor Dry Bulb / 55F+ Indoor Wet Bulb.
Payment: Please submit your completed Early AC/HP Replacement Rebate form to the address on the bottom right of this form. You will receive
one rebate check for the entire amount owed to you, providing any required QIV testing (see above) was completed on the new equipment. If the
testing cannot occur this year due to weather conditions, you will receive a $150 QIV Rebate after your COOL SMART QIV Trained Contractor
completes the test. Please allow 6-8 weeks for payment. Payment processing may take longer if information or documentation is missing from
the application.
Post Installation Verification: Sponsors reserve the right to conduct onsite verification to inspect that the equipment was installed according to
guidelines of the program. This verification is not to be construed as a safety review and should not be relied upon as one. If vendor determines
that installed equipment does not meet program specifications as described in the application, the vendor reserves the right to deny the rebate payment.

Energy Benefits: Customer agrees that the Energy Efficiency Program Provider (EEPP) has the unilateral right to apply for any ISO-NE capacity payments or
