Textbooks must be paid for in advance to Lifelong Learning and Special Programs (checks
payable to The State University of New York at Fredonia). They will be available, on site,
on the first night of class.
This is a challenge refresher course. You can challenge the course if you are a current EMT (within 18 months of
). There will be a written
expiration) or have been an EMT (any expired NYS EMT, regardless of when they expired
examination on the first night of class to determine what classes you MUST attend.
There will be a mandatory “Challenge Practical Skills Exam” on the second night of the class.
The student will be expected to attend all MANDATORY SESSIONS along with a CPR session. Failure to attend these
sessions will prevent the student from taking the State written examination. The student must produce ICS100/700
certificate and HAZMAT Awareness OSHA 1910 Compliance.
(The student can complete the HAZMAT
portion by attending a session scheduled for 3/3/16) If the student does not complete ICS 100/700 and
HazMat they will not be eligible to take the NYS Written Exam. The course is NYS Department of Health certified.
The NYS Written Exam will be given on 3/17/16.
The course will cover the new “Spinal Protocol”.

If you are being sponsored by a local agency, please note the agency’s name, address, and contact person on your
all textbook fees are due at the time of registration – agencies must provide their tax
registration form below –
exempt ST-119.1 form.
Further, if the sponsored participant withdraws from or drops the class, or is dropped for not
complying with attendance requirements, the agency will be responsible for paying The State University of New York at
Fredonia one-half of the NYS Department of Health, Bureau of EMS course reimbursement rate.
