Gift Raffle FUNDRAISER

Dear Friend/Supporter:

The National Council on Alcoholism and Drug Dependence – New Jersey (NCADD-NJ) is

planning to hold a Tricky Tray Gift Raffle Fundraiser on Friday, May 16, 2014 at the Hibernian

Club in Hamilton Square to raise money to support the organization’s efforts on behalf of

individuals and families affected by drug and/or alcohol addiction and mental illness.

We are asking for your support by making a donation today. Your monetary contribution, gift

certificate, merchandise, gift basket or other donation will help make our fundraiser a success.

Your donation will be featured at the Tricky Tray and your participation acknowledged in our

program-booklet at the event. Please complete and return the portion below with your donation.

Donations to NCADD-NJ are tax deductible! Our tax ID number is 22-2436069.

NCADD-NJ is a 501(c) (3) nonprofit organization founded in 1982 to advocate for those affected

by substance abuse. Among other programs, we sponsor the statewide Substance Abuse

Initiative/Behavioral Health Initiative that each year obtains treatment services for hundreds of

welfare recipients and DCP&P involved families with substance abuse and/or mental health

problems. In addition, NCADD-NJ collaborates with the NJ Juvenile Justice Commission to
