ORAL HISTORY

INTERVIEW CONSENT FORM

I hereby grant _________________________ permission to document through audio and/or

video recording and transcription oral history interview(s) for the purpose of preserving

historical information and heritage. The interview(s) may be transcribed and made available for

scholarly use. The information I agree to share with the interviewer is to be used for the purposes

of documenting the history of the Episcopal Church in the state of Alabama. When this material

becomes available, it may be read, quoted, or cited from and disseminated for educational and

scholarly purposes unless a specific restriction is made below.

This consent does not preclude any use which I may want to make of the information contained

in the recordings or transcription. I understand that I retain the ownership of all intellectual

property rights in the contents of the interview(s).

I understand that I may withdraw from the interview at any time. I understand that I have the

right to request that any tape made during the interview be given to me or destroyed or that no

transcript be made of its contents. I understand that I may stop the recording at any time to

discuss or clarify how I wish to respond to a question before proceeding.

Subject to the provisions below, upon completion of the interview(s) the tape and contents of the

interview will belong to the Episcopal Diocese of Alabama.
