The Trustees of Great Ormond Street Hospital Children’s Charity of Great Ormond Street
(1)
London WC1N 3JH a body corporate under Part VII of the Charities Act 1993 as trustees of
the Great Ormond Street Hospital Children’s Charity (registered charity no 235825) (the
‘Charity’); and
[Names and addresses of the Executors] (the ‘Executors’).

RECITALS
This Agreement is supplemental to the will (the ‘Will’) dated [date] [and any codicils] of

[name] (the ‘Deceased’). The Deceased died on [date of death] and a grant of probate was
issued out of the [Registry] Probate Registry on [date] to the Executors.
The Executors have not been able to locate the legatee [name] (the ‘Missing Beneficiary’)

OPERATIVE PROVISIONS

Definitions
In this Agreement, where the context admits, the following definitions shall apply.
The ‘Executors’ shall mean the Executors, or the survivor of them, and their

respective personal representatives and successors in title.
The ‘Claim’ shall mean a claim by the Missing Beneficiary or the Missing

Beneficiary’s Personal Representatives against the Executors arising out of the
Executors’ decision to distribute the estate if the Legacy had failed.
