INFORMATION SHEET
This is an information sheet only – the completed sheet will be used for the development
of an ‘Affidavit of Heirship’ by the Law Office of M. R. Carr
(Use Reverse Side if Additional Space is needed)
The final Affidavit will need to be signed, before a notary, by two witnesses who were
well acquainted with the deceased. Their names, and the number of years they were
acquainted with the Deceased, can be added upon signing (which can happen out of
state). The witnesses can be acquaintances or relatives, however they can not be
recipients of Deceased’s estate. After signing by the two witnesses, the heirs (spouse and
adult children as applicable) will also sign an attachment to the Affidavit.
