3FILING INSTRUCTIONS

3. The person submitting the Articles of Dissolution must complete the attached affidavit in which he or she
swears or affirms under oath, subject to a penalty of perjury, that he or she holds the requisite authority to
take this action. The affidavit must be notarized prior to submitting this form for filing with the Office of the
Secretary of State. Pursuant to Section 33-31-1402 (f) of the 1976 South Carolina Code of Laws, as amended,
the Secretary of State cannot accept this form unless this affidavit is completed.
4. A filing fee of $10.00, payable to the Secretary of State, must accompany this form.
