Part 18 Evidence checklist
Please tick to confirm the evidence you are providing with this form. If you do not provide all the

Income
Identity
Proof of income received by you, your
One document from List 1 and one from List 2
partner and anyone else living with you
for you and your partner, if you have one.
~ Five latest payslips, if paid weekly, the
last three if fortnightly or the last two if
List 1 – These must be original documents
paid monthly,
~ Current passport or travel document
~ If self-employed the most recent profit
containing a photograph, driving licence,
and loss accounts and, if applicable,
birth certificate or an EEA national identity
proof of any Business Startup
card.
Allowance
~ As proof of other types of income,
List 2
recent payment slips, bank or building
~ Adoption certificate, marriage certificate,
society statements showing amounts
divorce or annulment papers in your current
paid and the source, notification of an
name, letters from the DWP or HMRC, proof
award such as a court order
of immigration status from the Home Office,
~ For students, student grant, loan or
United Kingdom residence permit, recent
bursary notification letters and any deed
letter from a solicitor, social worker or
