Congratulations on your admission to New York University Study Abroad! We will reserve a space for you in the
program once we receive this completed confirmation form along with your non-refundable $500 deposit.
The deposit must be paid by check or money order made out to “New York University” and delivered to the Office of
Global Programs by mail or in person. This amount will be applied to your tuition bill once you have registered for
classes. Unfortunately, we cannot accept cash or credit card payments. All tuition deposits are non-refundable.
Please note that space in the program is limited. If the program reaches capacity before we receive your deposit, you
will be placed on a waitlist.

I agree that I am not currently on disciplinary probation or leave of absence with my current school and I
understand that if my discipline or enrollment status changes (including pending judicial action) before my
semester away I may be ineligible to participate.
I understand that I am responsible for obtaining my student visa and that, while NYU will support me in that
process by providing instructions and sample materials, I must adhere to the Consulate’s requirements and
deadlines.
I understand that I am responsible for completing the following forms prior to my departure: Foreign Travel
Release form, Medical History and Release form, General Information Form, Arrival Form, Housing
Information Form, and Insurance Form.
I understand that as an NYU Study Abroad participant I will be acting as an ambassador for NYU and
should conduct myself accordingly.
