Affidavit - Supplemental
Use this form to present the relevant facts that you did not know or forgot to include in your
previous Affidavit.
Before filing the Affidavit, make sure that you:
o Number all of your paragraphs consecutively (1, 2, 3, 4...)
o If you want to attach any document to your Affidavit, you must explain what it is in your
Affidavit and mark it as an Exhibit. For example, if you are attaching a copy of your tax
return, you would include as statement in your Affidavit which says “Attached as Exhibit
A is a copy of my 2009 income tax return”. Letter each one of your exhibits
consecutively (Exhibit A, Exhibit B…), and attach all exhibits at the end of your Affidavit.
o If your Affidavit, when all of the exhibits are attached, will be more than 25 pages long,
ask the Chambers Clerk or the Family Law Information Centre for help with numbering
your pages.
o Sign the Affidavit in the presence of a Commissioner for Oaths. The Chambers
Clerk may act as a Commissioner for Oaths. You will be asked to swear or affirm that
the contents of your Affidavit are true. You may also be asked for personal identification.
o Make 2 copies of the original Affidavit, including all exhibits (total of 3 with the original).
Copy the Affidavit AFTER it has been sworn by a Commissioner for Oaths.
o Once you have made copies, bring all copies of the Affidavit to the Clerks’ Office for
filing. You will receive 2 filed copies back.
Note: You cannot make any changes to your Affidavit once it has been sworn.
