1. Please enclose all relevant documents substantiating your claim as a person of Indian origin.

(Please also see Cols. 14 and 15).

2. Applicants must provide original and copy of landing paper.

3. Applications for children born to Indian parents in Canada must be accompanied by

a. The child's Canadian passport

b. Original birth certificate mentioning the names of the parents and

c. Both parent's Indian passport, PR Cards, Landing Papers, Citizenship Certificates

and copies of all these documents.
