Card, Landing paper, Citizenship Certificate and copies of the relevant pages of these

documents.

7. A Child should affix left hand thumb impression – right hand in case of female and left

in case of male (INSIDE THE BOX + applicant signature) + both parent will have to sign

under applicant signature.

8. Furnishing of false information will result in summary rejection of the application.

9. Note: Acceptance of application form does not confer any right on the applicant to the

PIO Card.
