I give my daughter permission to be a model in the American Girl Fashion Show presented by Lynn Meadows Discovery Center.
By signing this form, I am also giving permission for my child to be photographed and/or videotaped. These images will be
property of LMDC. I grant LMDC and American Girl Companies to use these photos or videos to promote LMDC and the
American Girl Fashion Show.
I release and hold harmless LMDC, the American Girl Companies and any other sponsors, volunteers, or staff for said event,
from any liability that may arise from my daughter's participation in the fashion show described above.

Submit your application by July 27 to avoid the wait! If you submit your application prior to the
deadline, your daughter will receive a specific audition time between 10am -3pm. Walk-ins are welcome
and accepted at 3pm! Early registration is not required. Please return application and signed release to
