__ last 6 months complete checking statements and/ or last 3 months complete savings

statements and/ or bank books for all accounts.

__ Employment letter on company letterhead

(Must indicate date of hire, number of hours work per week and rate of pay)

__ Last 6 consecutive pay stubs

__ Public Assistance Budget Letter

__ Social Security/ SSI Award Letter

__ Pension Award Letter

__ Verification of Unemployment benefits or most recent pay stub

__ Verification of Child Support, Alimony and/ or Disability benefits

__ Affidavits of Non Employment completed and notarized
