a reproduced copy of the Corporation's certificate of incorporation (and articles of amendment or
continuance, where appropriate) under the Business Corporations Act and a reproduced copy of the
Corporation's articles of incorporation, as approved by the Executive Director under Rule 154 and filed under
that Act, and
(b)
the prescribed application fee.
