Section 3: Overseas Student Health Cover (OSHC)
It
is an Australian Government requirement that all students studying on a student visa are covered by Overseas
Student Health Cover (OSHC) for proposed duration of their visa. Students who intend to bring their family members
to Australia are also required to provide evidence that their OSHC includes these family members.
MIT will process your OSHC payment with BUPA Australia. You will be advised on how to collect your OSHC card
during the orientation week.

Section 4: Statement and Declaration Regarding Your Personal Information
I, the Student, have read and understood MIT’s Privacy Policy located at

signing this Form, hereby authorise MIT to collect and use personal information, health information and sensitive
information about me in accordance with the Privacy Policy. Further, I understand and accept that:
MIT may collect and use information about me for the primary purpose of providing higher education services,
and undertaking related activities and functions;
MIT is required by law under the Education Services for Overseas Students Act 2000 and National Code of
Practice for Registration Authorities and Providers of Education and Training to Overseas Students 2007 to
collect and disclose information, both during and after my enrolment, ¸and must notify the Australian
government or designated authorities in certain circumstances including changes to my enrolment, failure to
pay tuition fees, breach of a prescribed condition of my visa and failure to comply with behaviour standards;
MIT may disclose information about me to MIT’s employees, consultants and/or agents, to my sponsor (if any)
and/or to third parties in connection with the provision of higher education services;
MIT may disclose information to overseas recipients (such as education agents), and MIT does not take steps
to ensure that such overseas recipients comply with the Australian Privacy Principles and MIT will not be
responsible for any failure by an overseas recipient to comply with Australian privacy laws, and having been
so informed, I consent to my information being disclosed to overseas recipients in such circumstances;
MIT may use and disclose information about me without consulting me first;
I can access information collected by MIT from me by contacting Student Administration at MIT
(admin@mit.edu.au).
