Instructions:

Make sure the whole palm is inked including the inter-digital area and follow the arrow as indicated for the direction of the palm print.

Press the hand starting from the start of the palm near the wrist all the way to the tip of the fingers as space on the form allows. Clean

the left index finger, re-ink and press the finger on the space provided following the direction of the arrow in the box. Ink the writers

palm area of the left hand and press the hand starting from the area near the wrist to the base of the little finger following direction of

the arrow in the box. Enter SID# & AFIS# if available, signature and date printed are required. Palm prints can only be submitted to

OSBI when accompanied with a ten print card from the same individual.
