Volunteer Agreement: As a volunteer with Loaves & Fishes you consent to:

1. Treat all guests, staff and other volunteers with respect

2. Honor the Loaves & Fishes 48 hour cancellation policy when possible

3. Closed toe shoes and appropriate clothes are required: tank tops and shorts/dresses ARE NOT ALLOWED

4. We reserve the right to have you not volunteer under our discretion.

5. Loaves & Fishes is not responsible for any lost/stolen items.

6. If you are not comfortable with any assigned task- please notify our staff.

7. You are required to stay the entire shift, unless prior arrangements are discussed.

8. Please be prompt when being dropped off and picked up.

Waiver and Release: Volunteer does hereby release and forever discharge and hold harmless LOAVES & FISHES

FAMILY KITCHEN and its successors and assigns from any and all liability, claims and demands of whatever kind or

nature, either in law or in equity, which arise or may hereafter arise from Volunteer’s work for LOAVES & FISHES

FAMILY KITCHEN. Volunteer understands that this Release discharges LOAVES & FISHES FAMILY KITCHEN from

any liability or claim that the Volunteer may have against LOAVES & FISHES FAMILY KITCHEN with respect to any

bodily injury, personal injury, illness, death, property damage, or any other liability that may result from Volunteer’s work

for LOAVES & FISHES FAMILY KITCHEN, whether caused by the negligence of LOAVES & FISHES FAMILY

KITCHEN or its officers, directors, employees, agents or otherwise. Volunteer also understands that LOAVES & FISHES

FAMILY KITCHEN does not assume any responsibility for, or obligation to provide, financial assistance or other

assistance, including, but not limited to, medical, health, Workers’ Compensation or disability insurance in the event of

injury or illness.

Photographic Release: Volunteer does hereby grant and convey unto LOAVES & FISHES FAMILY KITCHEN,

LOAVES & FISHES FAMILY KITCHEN’s sponsors and their agents all right, title, and interest in any and all

photographic images and video or audio recordings made during the Volunteer’s work for LOAVES & FISHES FAMILY

KITCHEN, including, but not limited to, any royalties, proceeds or other benefits derived from such photographs or

recordings. Volunteer grants permission to record, edit, transcribe, use, duplicate, modify, distribute, and/or publicly

exhibit the Volunteer’s presentation or appearance, and use of Volunteer’s name, likeness, voice, and biographical

information in any and all media now existing or hereafter developed, throughout the world, in perpetuity, without

restrictions or limitation

I HAVE CAREFULLY READ THIS CONTRACT AND FULLY UNDERSTAND ITS CONTENTS. I AM

AWARE THAT THIS IS A RELEASE FROM LIABLITY AND A CONTRACT BETWEEN MYSELF AND

LOAVES & FISHES FAMILY KITCHEN, AND I SIGN IT OF MY OWN FREE WILL.
