AS YOUR CHILD CHECKS OUT A
SCHOOL-OWNED INSTRUMENT...
Dear Parent(s) or Guardian(s):
We are very happy to be able to provide your band student with a "school-owned" instrument to use
for their band experience this year. Because this is an expensive investment we can only provide a
limited number of instruments for student use, instruments that, for the most part, are larger and cost
more.
In order for us to keep track of these instruments and protect this large investment of tax dollars, you
are being asked to read, complete, and sign a “MUSIC INSTRUMENT/EQUIPMENT
CHECK-OUT FORM.” This form will indicate exactly what is being issued to your band student
and provide you with an approximate value of the instrument. Please sign and return this form to
your band teacher immediately.
There is no rental fee for using this instrument but you are responsible for negligent damages that
occur beyond normal wear and tear. In order to avoid an unnecessary repair bill, we ask that you
adhere to the following guidelines while this instrument is in your students possession:
 Always transport and store the instrument in Its case. This really helps avoid accidental dents,
prevents any keys from being bent, and may save you a repair bill.
 Never let someone else play this instrument. In addition to not being sanitary, you'll be
responsible for any damages that may occur through misuse.
 Clean the instrument regularly as per directed by your band teacher. This will help to insure the
instrument will stay in top playing condition.
 If something is wrong with the instrument notify your band teacher immediately. PLEASE
NEVER TRY TO REPAIR IT OR HAVE IT REPAIRED YOURSELF, as this will only lead to
greater expense.
