JPSO BAND OF EXCELLENCE

BAND TRANSPORTATION AGREEMENT

2013-2014 SESSION

I am unable to provide adequate transportation for my child to participate in the JPSO Band of

Excellence Program and am hereby requesting that my child be allowed to participate in the Band

Transportation program. This does not include parents with transportation available to them.

I understand that my child will be picked up from a convenient location and shuttled to band practice by JPSO

BOE staff and that following practice, my child will be returned to their home.

I understand that I need to have my child at the designated location at the designated time so that I do not

interrupt the transportation of other students.

I understand that a parent or designated guardian must be at the designated location to accept their

children at the drop off point if this is the arrangement made with the transporting officer. If someone other

than a parent is picking up a child, or the location of pickup or drop changes, the transportation officer must

be advised in advance by calling Sgt. David Green at 504.655.7287 or Det. Rogers at 504.460.2574. (Read

instruction sheet to determine your transportation officer)

I understand that transportation officers are unable to stay at locations after the drop off time and that it is

imperative that a parent/guardian is at the drop off location for their child. I understand that if I am running late

and am not at the designated location at the designated time, my child will be transported to the next location

and must be met by a parent/guardian for pick up.

I understand that I must provide my child and BOE Staff with a valid and working telephone number

so that I can be contacted at all times that my child is in the care and custody of JPSO BOE Officials.

During the band session, band members may require transportation to and from various performances or events,

locations and venues.

This agreement also authorizes JPSO BOE staff to transport my child to these

performances with the same provisions outlined above.

By signing this agreement, I agree that I have read and understand the Band Transportation Agreement and

will totally comply with the transportation plan outlined above.
