If you do not have an Isle of Man driving licence (provisional or full), you must submit, with your application, a recent
passport type photograph which has been countersigned on the reverse by the person who will countersign this form.
Refer to the Notes section below, Item 6, for categories of people eligible to act as your counter-signatory.

Now endorse the rear of the photograph with the wording “I certify that this is a true likeness of (insert name of
applicant)” and add your signature and the date.
Notes
1.
This is a computer based test and, in view of the requirements for hazard perception testing, there is no paper
alternative available to candidates. When taking the test, answers can only be selected and de-selected using the
computer mouse, there is no option to use a keyboard.
2.
The test is made up of two parts, a multiple choice question paper and a hazard perception test. You must pass
both elements at the same test session to pass your theory test.
3.
You must pass the appropriate theory test for a car or a bike before you apply for a practical driving test for a
vehicle in the same group. (This does not apply if you intend to drive only a vehicle in group K, i.e. pedestrian-
controlled vehicles or mowing machines).
4.
The Pass Certificate is valid for a period of 2 years commencing on the date on which the theory test was taken.
5.
You may use your Isle of Man driving licence (provisional or full) as proof of identification in place of a
countersigned photograph.
6.
The person countersigning Part B of this form:
a.
must be resident in the Isle of Man
b.
must not be a member of your family (either a blood relative or connected by marriage)
c.
must be a Member of Tynwald, Minister of Religion, Advocate, Bank Officer, Civil Servant, School Teacher,
College Lecturer, Police Officer or Doctor or another professionally qualified person.
7.
You must be aged 16 years or over to apply for a test.
8.
You may not apply for a test if, at the time of application, you are disqualified from driving.
9.
Please supply details of any special needs or any form of reading/writing difficulty e.g. dyslexia or any other
reading difficulties you may have at the time you make your application by completing the appropriate section in
Part A of the form.
10.
If you need to cancel your test for any reason, you must give 7 days’ notice or you will lose your fee.
11.
If you fail the test you must wait 7 days before re-applying.
