DVR Statewide Service Agreement Amendment
1. This amendment is made by the Department of Workforce Development (DWD), on behalf of the
Division of Vocational Rehabilitation (DVR) and the identified Service Provider, parties to the DVR
Statewide Service Agreement dated July 1, 2016 to June 30, 2017.
2. The Service Agreement is amended as follows:
Section 8-Consumer Progress Reports, paragraph one, the following language was removed:
It is also expected that during the 2016-2017 contract period, all service providers will
transition to using the required reporting template for all DVR statewide services or submit
a reporting template of their own that closely follows DVR's format for review and
approval.
Section 8, paragraph one, the following language was added:
Reporting templates are occasionally updated and it is the responsibility of Service
Providers to be utilizing the most current. Reports for statewide services that do not use
the forms found at the DVR website will be automatically rejected without notice 60 days
after any updates have been made.

Section 8, paragraph two: DVR requires that all reporting meet the statewide technical

Section 8, paragraph eight has been revised to read:
Information regarding options for submission of reports is available on the DVR Service
Provider website for those who may need additional information.

Section 9-Payment for Services, paragraph five: All services in accordance with the technical
specifications and the DVR Statewide Service Fee Schedule require prior written
authorization by the DVR before services can be rendered and payment can be made.

Section 13-Assignment of Agreement to Subcontracted Agencies, paragraph two has been
revised to read: If there are new sub-contracted agencies that were not listed at the time the
2016-17 Service Agreement was submitted to DVR, the Provider must sign and return the
following forms to DVR: Certification Regarding Debarment, Suspension & Other
Responsibility Matters, Certification Regarding Lobbying, and Certification Regarding
Drug-Free Workplace Requirements:
