3. Standard Terms and Conditions:

Section 6.2 has been revised to read:

Maintain commercial liability, bodily injury and property damage insurance against any claim(s)

which might occur in carrying out this Agreement. Minimum coverage shall be one million dollars

($1,000,000) liability for bodily injury and property damage including products liability and

completed operations. Provide upon request motor vehicle insurance for all business

owned vehicles used in carrying out this Agreement. Minimum coverage shall be one

million dollars ($1,000,000) per occurrence combined single limit for automobile liability

and property damage.

4. All other provisions of the Service Agreement remain intact and whole.

5. By signing this Service Agreement Amendment, I agree that I am the legal representative for the

service provider indicated. I acknowledge and verify that I have read and understand all forms,

certifications and reference documents that comprise a service agreement with the State of

Wisconsin, Department of Workforce Development/Division of Vocational Rehabilitation and

agree to extend the 2016-2017 Service Agreement to 06/30/2018.
