Welcome to Forest Park!
We hope you are enjoying your residency in Forest Park and the City facilities and services are
meeting your expectations. Through the annual budget, your City Council and administration
strive to provide the best mix of services that can be obtained with available revenues.
As a new resident, you should be aware that the largest revenue source for providing City
services is the one and one-half percent (1.5%) income tax that is paid by all residents, businesses,
and wage earners in the City. The Forest Park Income Tax was authorized by a vote of the citizens.
The City Income Tax Department requires each resident, who is 18 years of age or older, to

make quarterly declarations and tax payments and to file an annual tax return by April 15
of
the year following each tax year. If you live in Forest Park but work in another community,
Forest Park provides you a credit of as much as 1.5% for earnings taxed by another City or
County. If payroll withholding is provided by your employer(s) and, if the withholding covers all
taxable income, then you need only to file the annual tax return.
To assist you in establishing a proper taxpayer relationship with Forest Park, and to help you
avoid possible interest and penalty charges, please complete and return this Tax Questionnaire
within the next fifteen days. Your tax file will be updated after you return your completed
questionnaire.
The Forest Park Tax Ordinance protects the privacy of your personal information and the Tax
Division staff will offer assistance when you call 595-5211. You are also welcome to come to
the tax office, located in the Municipal Building, between 8:00 a.m. and 4:00 p.m on Mondays,
Wednesdays and Fridays.
