COMBINED CERTIFICATION AND VERIFICATION AFFIDAVIT
ACCEPTABLE DOCUMENTATION
Under Article 17.5 of Title 8, Colorado Revised Statutes, as now existing or hereafter amended,
and Article 76.5 of Title 24, Colorado Revised Statutes, as now existing or hereafter amended,
the following forms of identification are permissible:
A Colorado driver’s license, Colorado driver permit, or Colorado identification card,
o
which is currently valid or which is expired for less than one year or a Colorado issued
temporary paper license which is currently valid or which is expired for less than one
year;
A U.S. Military identification card or military dependent’s identification card;
o
A U.S. Coast Guard Merchant Mariner card; or
o
A Native American tribal document.
o
If none of these forms of documentation are available from the Applicant, then under emergency
rules promulgated by the State of Colorado on July 31, 2006, the following forms of identification
are permissible until March 1, 2007:
Certificate of U.S. citizenship or Certificate of U.S. naturalization bearing Applicant’s
o
intact photograph;
Valid immigration document of:
o
•
Valid Foreign Passport with unexpired “Processed for I551” stamp or
with an attached “Temporary I551” visa;
•
Valid Foreign Passport with an “I-94” with an L1 or R1 status and a valid
Canadian driver’s license, or valid Canadian identification card;
•
Valid 1551 Resident Alien/Permanent Resident card; or
•
Valid 1688 Temporary Resident card, 1688B and 1766 Employment
Authorization card;
Certificate of naturalization bearing Applicant’s intact photograph and the raised
o
embossed seal of the issuing agency; or
Out of state driver’s license with the applicant’s photograph, or photo identification card or
o
photo drivers permit which is currently valid or expired less than one year from any of the
United States
COPIES OF DOCUMENTATION WILL BE MADE AND RETAINED IN THE APPLICANT’S FILE
