OFFICIAL JUNIOR LETTER OF INTENT for ages 10-14 & under

REGION POLICY FOR COMMITMENT TO A JUNIOR CLUB:

(The following statement needs to be read at the parents meeting prior to try-outs.)

"Completing this form commits said player to said club for the upcoming season. Commitment can only

be written. Failure to live up to the commitment will result in a player being ineligible for the upcoming

season unless commitment is waived by said club."

Commitment Forms may not be signed by players or parents/guardians (and

therefore are not binding) until Saturday, October 31, 2015 for players on teams in

the 14 and younger age divisions. This commitment form is for use when a player

has accepted a position with a team. The letter of intent cannot be signed until after a

tryout or acceptance of a position and with the knowledge of the exact age division and

team rank they will be assigned to.

If a player signs a letter of intent, they may not attend other tryouts.

After tryouts, completing this form commits said player to said club

for the 2016 season. Commitment must be written. Failure to live

up to the commitment will result in player being ineligible for the

upcoming season unless commitment is waived by said club.
