The CME Verification Form MUST BE SIGNED and SUBMITTED to AOFAS to receive CME credit.

CERTIFICATES verifying attendance for members and non-members will be made available following AOFAS receipt of this official

CME verification form indicating the actual number of hours being claimed for time spent in each of the meeting(s) below. Return

the completed, signed form to AOFAS Registration Staff on-site or the AOFAS Office. Registrants may login to

download their certificate of attendance approximately 3 weeks after the meeting. CME certificates will not be mailed.

INTERNATIONAL ATTENDEES: Certificates of meeting attendance may be picked up at the Registration desk.
