Instructions for Information Request (Form UCC11-A) (New Hampshire)

This form is for use in the State of New Hampshire.

Do not insert anything in the box in the upper right portion of this form; it is reserved for filing office

use

To assist filing officers that might wish to communicate with requestor, requestor may provide

information in item A. This item is optional.

Enter name and address of requestor in item B. This item is NOT optional.

Debtor name: Enter only one debtor name in Item 1; either an organization's name (I a) or an

individual's name (I b). This form will not be accepted if it refers to more than one debtor.

No searches for cross-reference possibilities. The State UCC Office conducts a search for only one

name per search form filed. The State UCC Office will not search for names that the filer might have

filed but did not. However, if State UCC staff notices another filing, staff may note a similar name in

the search report.

For example: A filing is made under Stearns, Steven d/b/a ''Steams' Home Improvement'' Later, a

search request is made for the name ''Stearns' Home Improvement''. If the searcher notices Stearns,

Steven d/b/a Stearns' Home Improvement, State UCC staff would indicate that it has found the filing

under a similar name, but requires a separate search request and fee to make such a search.

Only the first debtor name in a filing is indexed. Only the first debtor name on each UCC-1 financing

statement is indexed in the alphabetical file, unless payment has been made to index additional debtors.
