ADDITIONAL CONDITIONS

Petitioner shall maintain a driving log in which Petitioner must document in writing the time and
place of each departure and the time and place of each arrival. Said log shall be physically located
in the vehicle during any periods of operation and subject to examination upon request by a peace
officer.

If Petitioner is issued a traffic citation during the term of this occupational driver’s license,
Petitioner is required to notify this Court in writing within 48 hours of receiving the citation.

Petitioner shall not possess or consume any alcoholic beverages during the term of this
occupational driver’s license.

Petitioner shall not possess or consume any illegal drugs, controlled substances, or dangerous
drugs without a valid prescription during the term of this occupational driver’s license.

Petitioner’s driver’s license is suspended under Chapter 524 or 724 Texas Transportation Code.
Therefore, Petitioner is ORDERED to attend a Court‐approved program that is designed to provide
counseling and rehabilitation services to persons for alcohol dependence pursuant to §521.245
Texas Transportation Code. Petitioner shall provide proof of attendance to the Court no later
than 90 days after this Order is signed.
If at any time Petitioner violates any of these conditions, the occupational driver’s license granted herein
is immediately REVOKED.
EFFECTIVE DATE OF OCCUPATIONAL DRIVER’S LICENSE

Petitioner has not had a prior suspension arising from an alcohol‐related or drug‐related
enforcement contact as defined by §524.001(3) Texas Transportation Code within the five years
preceding the date of Petitioner’s arrest in this case. Therefore, this Order granting an
occupational driver’s license takes effect immediately and shall be valid until the end of the period
of suspension of Petitioner’s regular driver’s license unless revoked.

Petitioner’s driver’s license has been suspended as a result of an alcohol‐related or drug‐related
enforcement contact as defined by §524.001(3) Texas Transportation Code during the five years
preceding the date of Petitioner’s arrest in this case. Therefore, this Order granting an

day after the effective date of the
suspension listed above and shall be valid until the end of the period of suspension of Petitioner’s
regular driver’s license unless revoked.

Petitioner’s driver’s license has been suspended as a result of a conviction of an offense under
§49.04 ‐§49.08 Texas Penal Code during the five years preceding the date of Petitioner’s arrest in

this case. Therefore, this Order granting an occupational driver’s license takes effect on the 181
day after the effective date of the suspension listed above and shall be valid until the end of the
period of suspension of Petitioner’s regular driver’s license unless revoked.

Petitioner’s driver’s license has been suspended as a result of a second or subsequent conviction
of an offense under §49.04 ‐§49.08 Texas Penal Code committed within five years of the date on
which the most recent preceding offense was committed, this Order granting an occupational
driver’s license takes effect on the first anniversary (one year) of the effective date of the
suspension listed above and shall be valid until the end of the period of suspension of Petitioner’s
regular driver’s license unless revoked.
