Box a - The Kind of Payer box should always be completed.
The
"Medicare Employees Only" block applies only to the PR
Government employees not covered under Social Security but
which are covered for Medicare.
•
Boxes 1-16 - These blocks match the individual wage and tax
information blocks 8-24 of Form 499R-2/W-2PR.The totals
requested here must agree with the sum totals of all 499R-2/W-
2PR being included in your report. They should also match the
sum totals of Forms 941PR/943PR reported to IRS during the tax
year.
