Line 4. Enter the TOTAL AMOUNT YOU OWE by adding Lines 1, 2, and 3 above. Make
check payable to the San Francisco Tax Collector. Include your certificate
number on your check.
Section I. Not Renewing Your Registration / Other Changes
Business closed (or closing prior to June 30, 2005): Businesses that closed in 2004 (or
that will not be renewing for 2005-06) do not need to renew their registration. Check the
first box on Line I and indicate the final date of operation for the business in the space
provided. You must still file a final Payroll Tax Statement if your 2004 taxable San
Francisco payroll was $66,666.67 or greater.
Other Changes: If you need to make changes to your business account, complete and
return the "Request for Information Changes” form on the back inside cover of this
booklet.
LINES J, K & L: Payroll Tax Filing Information and Signature
Line J: If your 2004 taxable San Francisco Payroll Expense (see Page 10 for definition)
was $66,666.66 or less, check the appropriate box on Line J that applies to you,
skip past Line K and proceed to Line L.
Line K: If your 2004 taxable San Francisco Payroll Expense (see Page 10 for definition)
was $66,666.67 or more, check the box on Line K. In addition to submitting this
Business Registration Renewal form, you must also file the enclosed 2004
Payroll Expense Tax Statement.
Line L: Review the declaration, then sign and date the statement. Submit the entire form
in the envelope provided to the San Francisco Tax Collector with appropriate
payment, if any (along with your Payroll Tax Statement, if required). Be sure to
make a copy of this statement for your filing records.
