Student Acknowledgement Form
Please read each statement carefully. Your initials indicate that you have been informed of and fully understand
each policy/requirement of the EBCI Higher Education & Training Program.

1. _____ I understand that the deadlines for higher education funding are June 1
for the Fall

Semester, October 15th for the Spring Semester, and April 1
for the Summer Semester.
2. _____ I understand that if I make a grade below a “C-“, including a W (withdraw), an I
(incomplete), or a CS (continued study) or any other non-passing grade, I will be placed in
reimbursement status and must contact the Education Department to arrange for reimbursement
before I will receive future funding.
3. _____ I understand that if I owe the Education Department money for poor grades, etc. that I will
NOT be allowed to garnish future per capita payments nor continue to be funded.
4. _____ I understand that if I am in reimbursement status for 2 consecutive semesters, I will NOT be
eligible for Education Department funds for the next 2 semesters.
5. _____ I understand that if I receive a Pell Grant, any other federal or state grants or scholarships,
that they will be applied to my college tuition/fees/books/meals/housing bill(s) before any the
Education Department funds are applied.
6. _____ I understand that it is my responsibility to turn in my grades, semester class schedule,
account summary/history, book list and student bill BEFORE the Education Department will arrange
payment for the next semester. Grades are to be turned in within 30 days of the last exam for each
semester attended. If grades are not turned in within that time frame, the student will automatically
be billed. The department will not accept grades beyond this 30 day period.
7. _____ I understand that I may be funded for a maximum of 126 credit hours for my undergraduate
college career.
8. _____ I understand that it is my responsibility to inform the Education Department IMMEDIATELY
if I withdraw from one or more classes or if a personal crisis arises that affects my ability to
complete a semester.
9. _____ I understand that it is my responsibility to inform the Education Department if I have a
physical or mental disability or condition that may affect my academic performance PRIOR TO my
enrollment in an institution of higher education.
10. _____ I understand that funding from the EBCI Education and Training Program is dependent upon
tribal gaming revenues. Funding amounts may change without advanced notice at any time due to
changes in casino revenue amounts.
11. _____ I have received the memo from the Tribal Treasurer explaining the tax liability I am up
against and I understand that reconciling my taxes/liability is my responsibility.
12. _____ I understand that I will be given a copy of my funding budget from the Education
Department.
13. _____ I understand that it is my responsibility to pay my bills with resources provided by the
Education Department and to produce receipts upon request.
14. _____ EMAIL: The department will begin emailing its correspondence during the Fall 2013
semester. I understand I am responsible for keeping my email address current with the department.
It is my responsibility to check my email for education correspondence.
15. _____ The internship process has been explained to me via the “Internship Process Memo”. I
understand this internship program does not apply to out of area students.
