I understand that I must participate in showmanship, herdsmanship and the appropriate judging

contest for my project.

This is to certify that I plan to exhibit and sell the above item at the San Juan County Fair Market

Auction. I understand that my official 4-H Fair Entry Form is due on the date indicated on that form

and this form is to accompany the 4-H Entry Form and is also due on this date.

The undersigned, by virtue of our signatures, certify that: 1) We have read and have followed the

Premium Book sections and departments containing rules and regulations to my project and agree to

them concerning the Market Animal projects for 4-H members as stated. 2) If applicable to my project,

I also agree to provide the 4-H Livestock Sifting Committee the “Producer Affidavit and Market Health

Record”, “County of Origin” and the “Verification of Animal Age” forms for my project before my animal

is weighed at Fair. 3) I will immediately fill out and turn in a “Haul Tag” once my animal has been

deemed eligible by the Livestock Sifting Committee. 4) If available for my project, we have also

educated ourselves on what the Quality Assurance program is for my project and have followed these

practices in raising my project. 4) We are aware that failure to comply with these rules and regulations

may result in not being allowed to sell in the Market Animal Sale and/or future sales in San Juan

County.
