H. DOCUMENTS YOU NEED TO PROVIDE
When you submit this form, you need to provide certain documents (all information supplied to IBA will become the
property of IBA). Tick the boxes to indicate what you have included.
1. Confirmation of Aboriginal or Torres Strait Islander Descent Form (original document).
2. Proof of your income (for example, two recent consecutive payslips, a letter from your employer or, if
self-employed, a copy of your income tax returns for the last two years and income and expenditure
statements of the business for the last two years).
3. Evidence of your ability to pay legal and stamp duty costs (for example, a copy of recent bank,
building society, credit union statements).*
4. Evidence of your ability to provide the deposit or equity.
5. Details of current land or home ownership not financed through IBA (that is, a copy of your most recent
council rates notice, current loan agreement and your past six months of home loan statements).
* Not required for applicants interested in purchasing or constructing a house in a discrete Indigenous community where land title is held communally.

IBA respects your privacy and is committed to maintaining the privacy and accuracy of your personal information and to protecting such
information from loss, misuse, unauthorised access, alteration and destruction.

What will the information be used for?
IBA will use the information you have provided to assess your Expression of Interest and for any purpose incidental to or associated
with the administration of the Indigenous Home Ownership program. We may have to share your information with another person
or

Commonwealth or State or Territory government agency to obtain further information about you, which would broadly include:
• commercial and consumer information
• information on any overdue debts
• information on account status, credit worthiness, credit standing, credit history or credit capacity from another credit provider
• information from any state or local government rating agency and state or municipal utilities provider on the status of your
account with them
• income information, including any pension or other government benefits.
Storage of personal information
Information you have provided in this form and information we have collected about you for assessing your request (Personal
Information) will be stored in a safe place as required by Information Privacy Principle 4.

Your authorisation
By signing at the space/s provided below, you authorise us to collect and use your Personal Information in the way described under the
heading ‘what will the information be used for?’. You also authorise a person or entity described under that heading to receive from us
Personal Information, including any document containing Personal Information to satisfy the requirements of s 191(2) (a) and (b) of the
