Dog licenses are renewed in January. Please mail this form (or apply/renew in person) to Lisa at the Township

Office, 1621 Riverton Road. Tags will be issued by return mail for those licenses applied/renewed by mail.

Please include a self-addressed stamped envelope for return of license and tag.

The fee is $12 plus an additional $3 surcharge for dogs not spayed or neutered. Proof is required for spayed or

neutered along with proof of your dog's rabies vaccine is current and valid through November 2013. A $5 per

month late fee will be assessed for each dog license renewed after January 31st. Cat licensing is in January.
