1. Any emblem from the list above will be located at the far left of the plate and will use one space. If you selected an emblem plate, in the
blocks below, leave the first block blank.
2. The ISA takes up two spaces and must be located at the beginning of the plate number. Use the percent sign (%) to indicate that you
request an ISA.
3. State law requires the letters “DV” to be either the prefix or the suffix of your plate number. In the blocks below, insert the letters “DV”
in the either the beginning or the end of your personalized plate number.
4. In the remaining empty blocks below, complete your personalized pattern with your choice of letters numbers or symbols; each uses
one space. The available symbols include a period, a dash, a blank space, a Texas silhouette (use an asterisk (*)), a heart (use an “at”
mark (@)) and a star (use an ampersand (&)). You are not required to use all the spaces provided. (Example: DV AAA or AAADV)
5. Fill-in the blocks below exactly as you want your personalized license plate to look. You may list more than one option in case the
pattern you have chosen is already issued.
6. TxDMV will not approve a personalized selection that has already been issued, is offensive, or conflicts with an existing license plate
alpha-numeric pattern.
