diagnoses)

REASON (why are you working with this patient? clarify the type of work you do as well)

CURRENT ISSUES (focus on psychiatric symptoms and social stressors, only relevant

medical issues)

NEEDS (what you need from psychiatrist, what are questions for psychiatrist to answer)

HISTORY OF PRESENT ILLNESS (time frame, pertinent positives and negatives,

emphasis on psychiatric symptoms)
