All persons listed in Section 3 and Section 5 of this application must complete both a Garda Vetting Form and a

Good Repute Declaration Form. Additionally, if the applicant is a company or co-operative, a Good Repute

Declaration Form must be completed for the company or co-operative and signed by a director / company

secretary / member of the co-operative committee of management. These forms must be completed and

submitted, even if there are no convictions / infringements against the person or company / co-operative. A list

of relevant convictions / infringements is provided in Note A in the Guide.
