ANIMAL LICENSE APPLICATION INSTRUCTIONS

Any person within the city limits who owns a dog or cat over 6 months of age is required to obtain a license for their pet. Rabies

vaccinations are required at 3 months of age per state statutes and municipal code.

If submitting your application electronically to obtain a new pet license or to renew a pet license, please go to

and follow the on-line instructions. You may complete and submit all

necessary forms and payment through our on-line application.

If submitting by USPS, you may fill out this form on line and then print the application. Enclose the completed form, a

non-returnable copy of the rabies vaccination, spay/neuter verification (if applicable) and the corresponding fees

shown within the application fee table. Mail the envelope and entire contents with appropriate postage to:
