Guidance notes

A copy of this form should be completed by the pre-registration tutor or by the person who signed the employer declaration

on the PPSS application form:



Within three days of the pre-registration trainee starting their placement; and



Either on the first or fifteenth of each calendar month (as advised by PPSS) throughout the duration of the

placement; and



Following completion or cancellation of the placement.

In the absence of the pre-registration tutor the form can be completed by the Responsible Pharmacist.

email address as specified on the PPSS application form.
