Information & Requirement for Completing Form MV-18E
The MV-18E Affidavit must be submitted to request the correction of the vehicle identification number (VIN)
on a motor vehicle record, certificate of title and registration when the motor vehicle owner and insurance
company have determined that liability insurance coverage matches the vehicle identification number on the
vehicle but the title and/or registration for the vehicle reflects a different VIN number.
The MV-18E Affidavit and required documents can be submitted to your County Tag Office or the
Department of Revenue’s Processing Center-MotorVehicle. The mailing address for the Processing Center-

County Tag Office locations can be found in the government section of your local telephon directory or on
the Department’s website.
Most tag and title forms may be electronically completed and
printed from the Department’s website for signing and submission by mail or in person.
Form MV-18E must be completed and submitted as follows:
The following documents are required to correct a VIN (vehicle identification number or manufacturer’s
identification number):
If the vehicle requires a title , the following documents must be submitted to your County Tag
I.
Office or to this Department’s Processing Center-Motor Vehicle:
a.
Form MV-1, that has been completed in its entirety and signed by at least one of the
vehicle owners;
b.
The original Georgia title, that reflects the vehicle’s incorrect VIN;
c.
Form MV-18E, that has been completed in its entirety and signed by at least one of the
vehicle owners. The owner’s signature must be notarized. In addition to signing, the
notary must affix their notary seal or stamp and record the date their notary commission
expires.
d.
Form T-22B, that has been completed by a Georgia law enforcement officer; a County
Tag Agent or his or her designated employee, after making a visual inspection of the
vehicle’s VIN plate; or,
e.
A legible pencil tracing of the vehicle’s VIN plate (a/k/a serial plate).
If the vehicle does not require a title, the following documents must be submitted to your

County Tag Office.
a.
Form MV-1, that has been completed in its entirety and signed by at least one of the
vehicle owners;
b.
Form MV-18E, that has been completed in its entirety and signed by at least one of the
vehicle owners. The owner’s signature on this form must be notarized. In addition to
signing, the notary must affix their notary seal or stamp and record the date their notary
commission expires.
c.
Form T-22B, that has been completed by a Georgia law enforcement officer; a County
Tag Agent or his or her designated employee; after making a visual inspection of the
vehicle’s VIN plate; or,
d.
A legible pencil tracing of the vehicle’s VIN plate (a/k/a serial plate).
Any other available documents that will support the correction request should also be submitted.
There is no fee charged for the correction of the vehicle identification number. However, if there are any other
corrections or changes required or requested for a vehicle that is required to be titled, the title application fee of
$18.00 must paid.
Important Information:
The Department of Revenue and County Tag Agents reserve the right to deny
the correction of any motor vehicle record, certificate of title and registration. During the correction of a VIN,
if the Department or a County Tag Agent determines that a registration and/or title record exists for the
correct VIN, the owner must comply with additional requirements.
