INSTRUCTIONS
1. Every owner of the manufactured/mobile home must fully complete and sign this affidavit. Any missing
information will render the affidavit as incomplete and the title for the manufactured/mobile home cannot be
eliminated.
2. Kansas statute KSA 58-4214 requires
owner must sign this affidavit,

relation status between the names listed on the title or the assignment on the manufacturer’s statement of
origin (EX: and, or, &/or) has no relevance to this requirement. Owner(s) signature must be notarized.
3. Signature of the agent acting on behalf of the lien holder must be notarized.
If there is more than one lien holder, an additional affidavit (TR-63) MUST be completed for each lien
holder. ALL the affidavits and documentation listed in No. 4 below are to be attached/staple together.
Every owner of the manufactured/mobile home MUST sign ALL affidavits. If there is more than one lien
holder, attach the documentation listed in No. 4 to only one of the affidavits, TR-63.
4. The following documents must be attached to this affidavit when submitted to the Titles and Registrations
Bureau (T&R) for approval:
a) Kansas title* showing the owner(s) and if applicable, the lien holder’s name(s) of the front, or the
assigned manufacturer’s statement of origin, or an application for title process by a local DMV
office and;
b) A release of lien, if applicable, from the lien holder shown on the front of this form, and;
c) Copy of most recent receipts for property tax and real estate taxes as proof of payment.

If the owner of the manufactured/mobile home does not have a Kansas title or manufacturer’s statement of
origin, the owner may complete a Manufactured/Mobile Home Ownership Affidavit, form TR-64 and follow
the requirements listed on the form, then submit the TR-64 and supporting documentation and make
application for a Kansas title at the owner’s local county treasurer’s office.
5. The $10.00 fee
be attached to this completed form. Make check payable to the Kansas Department of
