This affidavit and inspection form must be completed and accompany an application for title in the following situations:
1) Anytime the owner of a vessel or outboard motor cannot obtain sufficient proof of ownership from the seller. (Depending on the latest
records on file with the Missouri Department of Revenue, the applicant may be required to obtain a court order to determine ownership.)
2) When applying for a replacement identification number or a new identification number for a vessel, outboard motor, or a manufactured
vessel trailer.
3) The first time application for title is made on a homemade vessel or outboard motor.
4) When the property or land owner applies for title(s) for vessels or outboard motors abandoned on their property without their consent.
The applicant’s portion of this form must be completed and signed by the applicant. A notary public must witness the applicant’s signature and
complete all notary information, only when obtaining a replacement identification number.
Who can complete the inspection portion of this form?
The inspection portion of this form may be performed by any law enforcement officer except that 1985 and newer model vessels must be
inspected by an authorized agent of the Missouri State Water Patrol, Missouri State Highway Patrol, or Missouri Conservation Commission.
Property and Land Owners — For an Original Title this form needs to be completed by an authorized inspector of the Missouri Highway
Patrol or Missouri Water Patrol. For a Salvage Title or Junking Certificate this form can be completed by any Missouri law enforcement officer
(Missouri Water Patrol, County Sheriff or local Police).
Title Correction — Any authorized law enforcement — Missouri State Highway Patrol (MSHP), county sheriff, police, etc. New VIN for
Manufactured Vessel
Trailer — Any authorized Motor Vehicle Inspector or officer of the MSHP — a member of the St. Louis City or County Auto Theft Unit — MSWP or
Missouri Conservation Commission.

Copy of the notification letter Notice to Owner(s) and Lienholder(s) Regarding Abandoned Vehicle, All Terrain Vehicle (ATV), Vessel, Watercraft,
Outboard Motor, and Trailer (Form 5227) mailed to the owner and lienholder;
r
Copy of the proof of notification to the owner and lienholder (signed green certified card, envelope stamped “return to sender” or a copy of the
completed postal receipt along with a copy of the Post Office’s Track and Confirm showing the letter was delivered or was returned to the sender;
r
Title and processing fee: Vessel $10.00, Outboard Motor $7.50, Vessel trailer $11.00; and
r
State and Local taxes based on the retail or fair market value indicated on the General Affidavit (taxes based on the address where the unit was abandoned).
