Instructions for Application for Extension of 90-Day Decal to 180 Days
Pursuant to Florida law, the purchaser must submit the application to the selling dealer within at least 60 days of the
purchase date, or the extension will be denied. Purchasers may submit the Application for Extension at the same time the
90-day decal is purchased from the selling dealer.
The purchaser must complete the purchaser information portion of the application.
The selling dealer is responsible for completing the selling dealer information portion of the application.
A check in the amount of $425.00 made payable to the Florida Department of Revenue must accompany the application.
The purchaser and selling dealer must both sign the application.
The extension decal must be affixed to the center of the original 90-day decal by the selling dealer. The extension decal
must be affixed before the 90-day decal expires.
