To be submitted by all applicants for a Schengen visa:

 Application form completely filled out and signed twice by applicant

 Valid travel document (valid at least 3 months after scheduled return)

 Copy of holder page travel document and previously obtained / refused visa pages

 Copy of ID card (if applicable)

 If from a third country: copy of residence permit or proof of legal stay

 Previous passport(s)

 One passport size photograph that meets Dutch requirements, in colour, not older than

 Copy of medical/travel insurance for Schengen

 Copy of flight reservation

Visiting family/friends, additional documents (translated in English):

 Completely filled out letter of guarantee / accommodation legalized by municipality,

from referent

 If referent is guarantor: form of guarantee plus proof of financial means (salary slips or

bank statements of the last 3 months)

 If applicant is paying: bank statements of the last 3 months

 Proof of relationship between applicant and referent

 Proof of social ties: NADRA family registration certificate, NADRA marriage certificate

 Proof of economic ties: letter of employment, salary slips (last 3 months), bank

statements (last 3 months)

Business / conference / cultural / sport event / short study, additional documents

(translated in English):

 Letter of invitation from Dutch company or organisation mentioning purpose, duration

and payment of trip

 If applicant is a business owner: NTN certificate 2 years, FBR proof of income tax

payment 2 years, bank statements last 6 months, proof of previous business dealings

with company (invoices and/or e-mails and/or purchase orders)

 If applicant is an employee: signed letter from employer mentioning purpose,

duration and payment of trip plus salary slips last 3 months

Tourism, additional documents (translated in English):

 Bank statements (last 3 months)

 Signed letter from employer with salary, employed since when and permission of leave

and mentioning duration of this leave

 Hotel reservation for entire stay in Schengen countries

Minors, additional documents in all the above cases (translated in English):

 Copy of birth certificate
