Sec. III: NETWORK TRANSFERS – Report how many pounds were distributed by your food bank
Food Banks: All products transferred from one regional food bank to another regional food bank in Arizona.
Food Bank Key: Arizona Gleaning Project; Community Food Bank; D; St. Mary's Food Bank Alliance; St. Vincent de Paul;
United Food Bank; Yuma Community Food Bank
A. Pounds: Food or non-food product distributed from any source
AZ Gleaning Project: Product distributed utilizing the services of the Arizona Gleaning Project when you do not know
which food banks are receiving product.
SUBTOTAL: Automatically calculates
DISTRIBUTION:
To Agencies: 501(c) (3) organizations, affiliated pantries, food banks, shelters, etc.
To Individuals: Individuals who “walk-in” to the food bank, or are provided food directly by the food bank or at food
banks’ offsite locations / branches.
SUBTOTAL: Automatically calculates
TOTAL NET DISTRIBUTED: Automatically calculates (adds subtotals except the Out-of-State Food Banks)
To Out-of-State Food Banks: Food shared with regional food banks outside of Arizona.
Sec. IV: DISTRIBUTION
Number of Households Served: Report the total number of households, the total number of individuals that have signed
the Household Distribution Site Sign-in Sheets.
Number of Individuals Served: Report the total number of individuals; add together the number of people in household.
The number will be greater than the number of households.
Number of Meals Served: Total number of meals served by all congregate sites for the month.
Emergency Food Boxes: Actual count of emergency food boxes distributed during reporting period, boxes which
include USDA commodities, donated or purchased product or DES funding to support distribution
CFSP Boxes: Actual count of boxes distributed
Food Cooperative Boxes: Actual count of food cooperative boxes sold, all sources, all locations
Sec. V: FOOD BOX DISTRIBUTION
TOTAL FOOD BOXES: Automatically calculates
Sec. VI: ENDING INVENTORIES LBS. - Physically located in the regional food bank’s warehouses
USDA Commodities: Food only
CSFP / Food Plus: Food only
TOTAL INVENTORY: Automatically calculates
Sec. VII: WASTE LBS.
Produce: Fruits and vegetables
Other Food: All food except produce
Other Product: Non-food products (i.e., cleaning products, diapers)
TOTAL WASTE: Automatically calculates
