INSTRUCTIONS:

1. Complete & return the signed 2016 ORV Permit Information Sheet / Agreement Card on the back of this form.

2. Enclose a photocopy of the current valid vehicle registration. If your vehicle is leased, a copy of the front page of the

lease agreement is also required.

3. If registering more than one vehicle, you may make a photocopy of the Agreement Card, and send it along with the

necessary documentation.

4. Enclose a self-addressed, stamped envelope so we can return the ORV Permit to you by mail.

5. Send a check or money order made payable to the Town of Barnstable (please do NOT send cash) for the appropriate

amount to the above address.

Issued to individuals with proof of residency in the Town of Barnstable. Proof is established by the address shown on your

vehicle registration. If your vehicle is not registered in the Town of Barnstable, you must also send:



A copy of a Barnstable tax bill, or a Year Round Residential Lease showing a Barnstable address.

Barnstable residents, your vehicle must be registered in your name (or the name of a person residing at

your same address) unless you have:



Written permission to use a company vehicle, or proof of ownership of the company on the

vehicle registration.



A minimum one year vehicle lease agreement (front page only) if applicable.
