4Instructions for Completing the APSO Nomination Form
Deadline for submission: 5:00 p.m. (EST), Friday, June 6, 2014
General Information
Nomination forms will be accepted from schools in the following College Board regions: Middle States,
Midwestern, New England, Southern, Southwestern, and Western.
•
We are unable to consider students living outside the United States.
•
We can accept one application per school.
•
The nominee must be entering 11th grade in fall 2014.
•
If you are handwriting the application, use print letters; cursive is illegible on fax transmissions.
Part 1: For the Student
•
Attach a biographical essay (200–400 words) that describes your educational, extracurricular, and civic
experiences and explains how these experiences make you uniquely qualified to represent your peers as a
member of the Advisory Panel on Student Opportunity.
•
Do not include resumes, transcripts, or photos.
•
Check the appropriate boxes if you have used any of the indicated College Board programs or services. If
you have no experience with any of them, check "No Experience.”
Part 2: For the Counselor or Teacher
Attach a one-page statement explaining why your nominee would excel at representing students from your school
and from across the nation. Please include an example of his/her insight into educational issues impacting today’s
students and describe how long you've known the student and in what capacity.
Part 3: For the Parent/Guardian

Read and sign pages 1, 2 and 3 of the nomination form.
•

How to Submit the Application
Send completed three-page application, biographical essay, and one-page statement by mail, fax, or email. Do not
send duplicate copies. You will receive an email confirming receipt of your application. The nominee and
counselor/teacher will be notified of the status of the nomination via email on Monday, August 18, 2014,
