Please complete items 1‐18 on the following page and send this sheet to your adviser at OIA. Please include copies

of the items listed below with your personal information sheet. Our mailing address is Office of International

Affairs, The University of Chicago, 1414 E. 59th Street, Room 291, Chicago, IL 60637, USA. Scanned photocopies via

e‐mail are preferred.

□ Your current curriculum vitae.

□ Photocopies of your I‐94 Departure Record card, front and back (even if the back is not marked).

□ Photocopies of the identification pages of your passport.

□ Photocopy of your current U.S. visa in your passport.

□ Photocopies of your three latest pay stubs.
