Membership Form
The PTO is an organization made up of parents, teachers, school staff and community members. The purpose of
this group is to cultivate a partnership between home and school for the benefit of all students. We will establish
traditions to involve parents, teachers, the community, and the children in a safe, fun and learning environment.
Joining PTO is a fun and easy way to support your school. Please become a member by filling out the information
below. The membership fee is only $5.00 per family. Membership does not require meeting attendance,
(although we encourage your attendance), your membership gives you voting privileges at meetings. Return this
form along with the membership fee to the school office. By just paying your membership fee you have already
made a difference.
